Dear All, Yesterday, we shared a horror story about Nigeria’s primary health care (phc) system. Today, we are glad to share a positive response to the challenge of how to strengthen PHC in Nigeria being piloted by the partnership of KTU, BMJ, HRI West Africa.


'STRENGTHENING PRIMARY HEALTH CARE SYSTEM IN NIGERIA
Weak health system results from several factors including  poor medical knowledge and poor clinical competence which are
major causes of death and suffering in resource poor settings. Misdiagnosis, under treatment, over treatment, polypharmacy, other irrational drug use, practitioner denial and cover-up, are also consequencies of weak health systems in Low and middle income countries. In addition parents and caregivers often lack the basic information they need to make life-saving decisions in medical emergencies. Medical professionals and other cadre of health workers often lack the necessary training to deliver effective care and rarely use guidelines when they should.
The World Bank Nigeria Service Delivery Indicator (SDI) survey report on primary health care (PHC) in Nigeria, in 2011, particularly the section on clinical competence as judged for  both ‘diagnosis accuracy rate (37%) and treatment accuracy rate (17%) was catalytic in our creating this PACK (Practical Approach to care Kit) Nigeria Adult guide pilot. The partnership of KTU/BMJ/HRIWA are currently piloting PACK Nigeria Adult guide in 52 PHCs under the SPHCDAs in three states (Adamawa, Nasarawa, Ondo) aimed at improving the clinical competence of the users, particularly diagnosis and treatment accuracy rates. The partnership is supported by NPHCDA, FMOH and World Bank Nigeria.
The Practical Approach to Care Kit - PACK Nigeria Adult guide - is a comprehensive, evidence based guide which has been localized in Nigeria for Nigeria. It is integrated for all primary health care clinician cadres (JCHEWs, CHEWs, CHO/Nurse/Midwife, Medical Officer) to use to support primary health care of the adult 18 years or older. PACK Nigeria child will be introduced separately. PACK uses simple algorithms to evaluate and treat the patient with common symptoms and a standardised checklist format to care for the patient. The guide covers 40 common symptoms and 20 chronic conditions seen in PHC in Nigeria. It supports the clinician to integrate the care of the patient with multiple problems and wherever possible prompts the diagnosis of priority conditions. It promotes a two-way referral system from PHC to secondary and tertiary levels of care.''
